HEALTH INSURANCE PLANS
We are preferred providers for most health insurers and most insurance companies provide coverage of physical therapy services. As a courtesy to you, our staff will verify your physical therapy benefits during your first appointment or you may contact your insurance company directly. Verification of benefits is not a guarantee of payment. You are responsible for your insurance coverage and payment on your account. We encourage you to learn the requirements and restrictions of your particular plan. Common requirements and restrictions of some plans include:
- Referral from a primary care physician
- Co-payment / Co-insurance at time of treatment
- Annual monetary or number of visit cap on services
- Deductible responsibilities
We file insurance claims directly to your insurance company. Co-payments / Co-insurance are due at the time of treatment and our front office staff will be happy to answer any questions you may have regarding your account.
WORKER'S COMPENSATION
If you were involved in a work-related accident or injury and have a current worker’s compensation claim, you may be eligible for physical therapy. Most worker’s compensation companies require a physician referral for physical therapy and approval must be granted by the insurer prior to treatment. We currently accept all patients requiring rehabilitation for a work-related injury.
NO INSURANCE | CASH PAYMENT
At Complete Body Physical Therapy we believe that no one should be denied physical therapy due to lack of insurance coverage. Our clinic offers a discounted cash rate to those who do not have appropriate insurance coverage. Payment will be required at the time of service unless other arrangements have been made. MOTOR VEHICLE ACCIDENT / PERSONAL INJURY
We work with all individuals who have been involved in an auto accident or personal injury. Please contact our office staff regarding billing questions since each case is different. Physical therapy services as the result of an auto accident or personal injury typically require a referral from a physician prior to beginning rehabilitation.
